Dear parents and carers, 

The school nurses will be visiting the school on Wednesday 2^nd March to offer students aged 12 + the chance to have their first or second dose of the Prizer Covid-19 vaccine.

For students over the age of 16, they are able to self-consent if they wish to have the vaccine so they can complete the attached consent form instead of using the e-consent form. 

Any completed forms can be returned back to the school.
